Your most recent eye examination should not be more than 2 years old. If your last eye examination was more than 2 years ago, we advise that you obtain an up-to-date prescription prior to purchasing your RX watershades. If you are planning to visit your optometrist after placing this order, answer 'Yes' to 'Is your prescription less than 2 years old' and proceed with the rest of the form.

Single Vision prescription lenses are for those that need correction for near or distance vision.  Varifocal or multifocal lenses correct vision for both near and distance. LiP's RX varifocal lenses are progressive so they have no visible line on the surface of the lens.

The lens material for Typhoon, Surge and Flo RX is NXT™ (Trivex).  All RX lenses have anti-reflective, anti-scratch, hydrophobic and oleophobic coatings as standard.

Standard clear lenses are tintless, non-polarized and offer 100% UV protection

    PLEASE READ...

After you place a varifocal order, we will send you a sample frame. When you receive this, please contact your optometrist for an eye exam and attend with the sample frame to obtain an accurate reading for your segment height. This reading can only be taken with the frame as a reference point. At the same time you may need to request a 'Near PD' reading if this is not recorded on your prescription. When you send us the segment height reading, we can make a start on the lenses. The sample frames can be returned to us after the measurements are taken.

    ANTI-FOG COATING

Our ORION-X anti-fog lens coating works in combination with the double vortex venting system. This permanent coating is applied to the inner lens surface only and works in tandem with the ORION-X activation cloth (supplied). It is designed to counteract fogging caused by body heat or moist exhaled breath that becomes trapped behind the lenses.

    YOUR SPHERE READING

Please tell us if you have a positive sphere reading in either your right, your left eye or in both eyes? If you have a positive sphere reading in either of your eyes, a lens thinning charge will apply for both lenses

    RIGHT EYE

Add together your SPH + CYL readings for your right eye, and indicate the combined value in the choices below.

    LEFT EYE

Add together your SPH + CYL readings for your left eye, and indicate the combined value in the choices below.

RIGHT EYE (OD) - add your RX readings for the right eye

The sphere (SPH) indicates the strength of lens required to correct your focus. A positive value corrects long-sightedness (hyperopia), whereas a negative value corrects short sightedness (myopia). We can accommodate SPH readings of (+5) to (-8).

The CYL (cylinder) value denotes the strength of the astigmatism. Astigmatism means that the eye has difficulty focusing at certain angles. If you have a cylinder value, then you will also have an Axis value. If your prescription shows "DS", this means that you do not have an astigmatism, and you should leave the field blank.

The Axis is the angle of the astigmatism correction and specifies where the astigmatism is on your eye. The measurement is in Degrees. The values range between 0 and 180. If you have a "Cylinder" value, then you must also have an Axis value. Please add it here or leave blank if you have no Axis reading.

ADD is the extra magnification required to help see objects near to you and is required only for varifocal lenses. Please ignore for single vision lenses.

LEFT EYE (OS) - add your RX readings for the left eye below

The sphere (SPH) indicates the strength of lens required to correct your focus. A positive value corrects long-sightedness (hyperopia), whereas a negative value corrects short sightedness (myopia). We can accommodate SPH readings of (+5) to (-8).

How many PD (Pupil Distance) numbers are on your prescription?

The Pupil Distance (PD) is the distance between the centre of your pupils. Your PD is important in the lens-making process because it tells us where to centre your prescription in the frame you chose.

We require your PD Distance reading before we can proceed with preparing the lenses. Your optometrist will hold this information on your file.  Please contact them and send this information to us after you have placed your order.

    PD - NEAR VISION

The Pupil Distance (PD) is the distance between the centres of your eyes. Your prescription may have one reading, two readings (right eye / left eye) or no readings.

    PD - DISTANCE VISION

Please add your PD 'Distance' reading(s) here.

    PRISM & BASE

Prism / Base are readings for a lazy muscle and are not present on most prescriptions. If there are measurements for prism & base on your prescription, please enter them in the boxes below. Otherwise, please leave empty.

    Left eye

*** Please note that the price indicated below is an estimate. We will send you a confirmed price once we have reviewed your prescription. If there is any change to the price you need to pay, we will let you know by email. Alternatively, if you would like a confirmed price, please choose "I would like to enter it now" above and add your prescription readings. ***
